A dedication hole is the perfect way to recognize your loved one(s) and give perspective to our many golfers, volunteers and spectators. As our objective is to raise awareness, we want to provide more real life stories in hopes that lives are saved simply by just being aware earlier. Complete this form to dedicate a hole on the tournament golf links to yourself or your loved one(s) affected by or lost to cancer.

Part of the Proceeds will be donated to our grant beneficiaries, which are all 501(c)(3) organizations providing various resources to those in their battle with cancer. Each dedication hole sponsorship is $25.00 and are valid for one tournament.

Use the "Dedication in honor/memory of" text box to provide the name(s) of the individual(s) to which you are dedicating a hole; and please note if the named person is a survivor or lost loved one.
